If you would like to bill insurance please contact our office prior to your visit to ensure we have your updated insurance information. Our team does our best at confirming your benefits coverage, however, for some plans the information is not available. We recommend you reach out to your insurance company to ensure the provider you are seeing and the service you are requesting is covered and if there are co-pays, co-insurance or deductibles that apply. 

Patients needing to cancel their appointments should do so at least 24 hours in advance or else they will be billed a $60 as cancellation fee for office visits and $100 cancellation fee for procedures. For patients who are covered by medicaid, federal law protects you from incurring cancellation fees for covered services, therefore our clinic has a strict 3 strikes policy for late cancel and/or no shows. If you reach the third strike our clinic will refer you to another clinic for your care. Thank you for understanding and respecting your providers time.
