Our providers are contracted with several insurance companies. The list below may not include all contracted companies, please inquire with our office if you have questions.

• UMR and United
• Pacific Source 
• Oregon Health Plan 
• Care Oregon (billed out of Network)
• Healthshare Providence 
• Healthshare OHSU require primary care provider referral. 

Please call our office or your insurance company to ensure the provider you will be seeing is in contracted.

As always, if we do not accept your insurance, we are happy to provide you with a superbill to submit to your insurance plan for reimbursement. 

*Note: It is your responsibility to determine if your insurance is active and your provider is in-network and your benefits summary as well as deductibles and out of pocket limits. If your provider is out of network, check to see if your insurance offers reimbursement for out of network provider cash payments.
